We are a clinician owned Skilled Home Healthcare Company seeking a dedicated and knowledgeable Physical Therapist (PT) to join our team. The ideal candidate will possess a strong understanding of anatomy and patient care, with experience in both clinic and home health settings. This role involves developing individualized care plans, conducting assessments, and implementing effective treatment strategies for patients in various stages of recovery. The PT will work collaboratively with healthcare professionals to ensure optimal patient outcomes.

Duties will include:

• Conduct comprehensive evaluations of patients to determine their physical therapy needs.
• Develop and implement personalized treatment plans based on patient assessments and goals.
• Utilize various therapeutic techniques to enhance patient recovery.
• Provide education and support to patients and their families regarding care plans and home care strategies.
• Collaborate with multidisciplinary teams to facilitate discharge planning and ensure continuity of care.
• Maintain accurate documentation of patient progress and treatment outcomes in compliance with agency standards.
• Participate in ongoing professional development to stay current with industry best practices.

Qualifications:

• Strong knowledge of anatomy and physiology relevant to physical therapy practice.
• Previous experience in a clinical setting, preferably with exposure to acute care environments.
• Familiarity with developing care plans tailored to individual patient needs.
• Experience in home health or community-based care is highly desirable.
• Ability to work effectively within a team-oriented environment while also demonstrating independence in decision-making processes.

Pay: $16.20 – $17.35 per hour

Expected hours: 1 – 30 per week
