Upon receiving a referral from our trusted sources, we immediately set in motion a comprehensive process. This includes gathering all necessary information, such as face-to-face visit notes from doctors and a detailed patient history. This meticulous preparation ensures that we can deliver the best-skilled care in the home, including PT, OT, skilled nursing services, and speech therapy if required. We also offer skilled home health aide assistance for improvement with ADLs and bathing while the patient is receiving skilled services and Rehab in the home.

Should the patient encounter challenges upon returning home, we have a solution in place. We have established partnerships with several personal care companies, ensuring that our patients can receive immediate assistance. Our skilled therapy team and nursing team are also trained to effectively train any personal care aids or other caregivers in the home. This proactive approach helps to reduce the patient’s risk for hospitalization.

We also offer medical social work Services to help patients plan for a change in insurance or add additional insurance, looking to state—— or city-funded using advice needs for the patient and family we’re helping.
